This form is for non-OFC members to register for our summer camp sessions. OFC members should have received a registration form via email. If not, please reach out to admin@olympianfencingclub.com.

Please submit one form for each camper attending. Once you submit a form you will receive a follow up email from our administrator with payment links. If you do not hear from us within 2 business days please email admin@olympianfencingclub.com.

OFC Training Camp Waiver

OLYMPIAN FENCING CLUB is a member of the United States Fencing Association (USA Fencing) with its certified insurance.

UPON ENTERING EVENTS SPONSORED BY USA FENCING AND/OR ITS MEMBER OLYMPIAN FENCING CLUB, I AGREE TO ABIDE BY THE RULES OF USA FENCING, AS CURRENTLY PUBLISHED (WWW.USFENCING.ORG). I UNDERSTAND AND APPRECIATE THAT PARTICIPATION IN A SPORT CARRIES A RISK TO ME OF SERIOUS INJURY, INCLUDING PERMANENT PARALYSIS OR DEATH. I VOLUNTARILY AND KNOWINGLY RECOGNIZE, ACCEPT AND ASSUME THIS RISK AND RELEASE THE USA FENCING, THEIR SPONSORS, EVENT ORGANIZERS AND OFFICIALS FROM ANY LIABILITY.

EXPRESS CONSENT FOR MEDICAL TREATMENT

I EXPRESSLY GIVE MY CONSENT FOR THE STAFF AND COACHES OF OLYMPIAN FENCING CLUB TO OBTAIN MEDICAL CARE FROM ANY LICENSED PHYSICIAN, HOSPITAL OR CLINIC FOR ANY INJURY OR ILLNESS THAT MAY ARISE DURING ACTIVITIES ASSOCIATED WITH OLYMPIAN FENCING CLUB. IN THE EVENT THE PARTICIPANT IS A MINOR, I HEREBY EXPRESSLY GRANT OLYMPIAN FENCING CLUB PERMISSION TO ACT AS GUARDIAN FOR THE MINOR PARTICIPANT IN OBTAINING REASONABLE EMERGENCY MEDICAL TREATMENT FOR ANY INJURY OR ILLNESS THAT MAY ARISE DURING THE ACTIVITIES ASSOCIATED WITH OLYMPIAN FENCING CLUB.

EXPRESS CONSENT FOR PHOTOGRAPHY

I EXPRESSLY GIVE MY CONSENT FOR THE STAFF AND COACHES OF OLYMPIAN FENCING CLUB AND/ OR THEIR DESIGNEES TO TAKE PHOTOGRAPHS, VIDEO OR AUDIO RECORDINGS OF THE UNDERSIGNED AND ABOVE MENTIONED PARTICIPANT TO USE FOR THE PURPOSE OF PROMOTING OLYMPIAN FENCING CLUB ON THE OLYMPIAN FENCING CLUB WEBSITE AND/OR OTHER PROMOTIONAL MATERIALS.

Step 2

All campers must have either a temporary or an annual membership with USA Fencing. The trial membership is free and lasts 90 days. This is a great option for first time campers. Please use this form to sign up for a free trial membership before camp starts, but not before the 90 days expires. We’ll include a reminder a week before camp starts. Please select “Olympian Fencing Club” as your club designation.

Campers who need to obtain an annual membership at the “Access Level” can do so at https://member.usafencing.org/
